Clinical Trial Studies New Treatment For High-Risk Heart Patients

A clinical trial under way at the East Carolina Heart Institute is testing the benefit of a new medical device designed to help high-risk people suffering from leaky heart valves.

Dr. W. Randolph Chitwood Jr., director of the Heart Institute and professor of cardiovascular surgery at the Brody School of Medicine at East Carolina University, secured the clinical trial for the MitraClip System for the treatment of mitral valve regurgitation (MR). A serious heart condition, MR can lead to arrhythmias and congestive heart failure if left untreated.

Dr. Curtis Anderson, a cardiothoracic surgeon and assistant professor of cardiovascular sciences, and Dr. Ramesh Daggubati, a cardiologist and clinical professor at Brody, collaborated to perform mitral valve repairs on two patients at Pitt County Memorial Hospital without making the traditional incision in the center of the chest to access the heart. Chitwood joined Drs. Rajasekhar Nekkanti and John Cahill, ECU cardiologists, to assist in the procedure.

In each case, the multidisciplinary team of cardiologists and surgeons placed a thin catheter in the patient’s groin vessel, guided a tiny clip through the heart’s septum and placed the clip between the leaflets of the valve to stop the leak.

“With mitral insufficiency, we normally use the heart/lung machine to either repair or replace the valve,” Chitwood said. “This new procedure is catheter-based, and can be used in high-risk patients who wouldn’t do well on the heart/lung machine. These patients have severe symptoms stemming from the leaky valve and can’t be operated on safely any other way.”

Chitwood said this new procedure “gives the East Carolina Heart Institute an entirely new set of tools for mitral valve repairs in high-risk patients. It is the least invasive way that we can fix a valve. Centers selected to perform these procedures have demonstrated expertise in applying new medical technology under FDA (Food and Drug Administration) guidelines.”

With the clip, patients leave the hospital the next day and within two or three days return to normal activities – or, in many cases, better-than-normal activities.

“Most of the patients are very sick,” said Daggubati, a clinical associate professor of cardiovascular sciences. “Medical management is the only option for them because surgeons consider the risk too high and the patients inoperable.”

Anderson said patient selection is the key to a successful outcome.

“I think a lot of patients with heart failure will be attractive candidates for this device,” he said, adding that collaboration among physicians is vital. “We want patients to have the confidence of knowing that cardiologists and cardiac surgeons are working side by side to give them the best possible outcomes.”

The mitral valve is a one-way valve that connects the left atrium to the left ventricle of the heart. With mitral valve regurgitation, the valve does not seal completely, and blood leaks back into the left atrium. This reversed flow can cause heart and lung damage, as well as death. Symptoms may include an audible heart murmur, shortness of breath and heart palpitations.

Failure to Investigate Cosmetic Surgeon Can Cost You Your Life

A recent death illustrates the importance of investigating your cosmetic surgeon, says Dr. Mark Scheinberg from the Center for Cosmetic Laser Gynecology. Lidvian Zelaya, 35, underwent a procedure for abdominal liposuction and buttocks injections at the Strax Rejuvenation Clinic in south Florida.

She was rushed to the hospital, but died only a few hours after the start of the procedure. Investigations of public records by ABC News reveal that Dr. Roger L. Gordon, Zelaya’s plastic surgeon at Strax, has had 20 lawsuits filed against him since 2004. He was placed on probation by the Maryland Board of Physicians for undisclosed reasons and he had his staff privileges terminated by the Florida Medical Center. The Sun Sentinal revealed that Gordon was disciplined by the state in connection with two deaths in 2004.

“If you’re choosing a plastic surgeon, you want to do your homework,” says Scheinberg. “You’re entrusting them with your body – and your life. You need a doctor you can have confidence in.”

Scheinberg has been board certified in obstetrics and gynecology since 1981 and has performed over 10,000 surgical procedures. Scheinberg has received 100s of hours of training with Professor Adam Ostrzenski, M.D. Ph.D., a pioneer in safe, effectivevaginal reconstruction and cosmetic vaginal surgery.

Zalaya’s cause of death is under investigation, but she was declared healthy enough for surgery by doctors at the Strax Clinic. “I think it’s an inherent conflict of interest if you are getting screened by the surgeon who wants to do the procedure,” said Spencer Aronfeld, a malpractice attorney retained by Zalaya’s husband, Osvaldo Vargas.

Vargas has been publicizing the case as a way of warning others to investigate their cosmetic surgeons. In particular, he is urging patients to get a second pre-operative examination by an independent doctor. He said that Zelaya chose the Strax Clinic because she got a good deal on the procedure.

“You don’t want to rush into this kind of decision or select the doctor who offers you the lowest cost,” notes Scheinberg. “You want someone who has a long record of success with the procedure you are getting. Complaints and disciplinary actions against doctors are part of the public record. Patients can and should look this information up.”

New UN Program Will Ensure Developing Nations Continue to Have Access to Drugs Effective At Treating Malaria

Earlier this week, the United Nations announced a new plan of action designed to help fight disease resistance to artemisinin, the world’s most potent treatment for malaria. Launched by the UN World Health Organization (WHO) and the Roll Back Malaria Partnership (RBM), the groups hope to better contain and prevent resistance to artemisinins, the critical component of artemisinin-based combination therapies (ACTs), which are currently the most effective weapon in treating falciparum malaria.

The WHO’s announcement describes the plan as a five-step process which “aims to contain and prevent artemisinin resistance by stopping the spread of resistant parasites, increase monitoring and surveillance for artemisinin resistance, improve access to malaria diagnostic testing and rational treatment with ACTs, invest in artemisinin resistance-related research, and motivate action and mobilize resources.”

Global Health Progress is hopeful that this action plan will help to ensure that developing nations continue to have access to drugs that most effectively treat malaria. Many malaria-stricken countries lack the resources needed to keep an effective health care system running smoothly. Strong, effective health systems are vital to helping those in need of access to drugs, and health care improvements made today will strengthen the ability of countries to develop sustainable systems that can meet tomorrow’s challenges. Stopping the spread of drug-resistant strains of malaria and other diseases are essential to these goals.

Global Health Progress Commends European Union and India Agreement Increasing Access to Drugs in Developing Countries

According to a Reuter’s article, the European Union (EU) and India have come to an agreement which should ease the process of exporting Indian drug manufacturers’medicines to developing countries, increasing these countries’ access to drugs.

Karel De Gucht, commissioner of the European Union (EU) has confirmed an agreement between the EU and India that will resolve some of the disputes that lead to the seizure of generic drugs in transit last year. This agreement will allow shipments of medication to pass through Europe to developing nations without being checked for anything except counterfeiting.

The new agreement will also amend custom codes allowing shipments of medications from India to reach countries located in South America via Europe. The confirmation comes amid trade talks between the EU and India.

Global Health Progress commends the agreements between the European Union and India, which will increase access to drugs, primarily generic medication, for underdeveloped countries. This process affects the affordability of generic medications to underdeveloped countries which fight the prominence of infectious diseases such as HIV/AIDS and malaria.

The Exercise Myths

Global Health Progress Applauds HIV-Prevention Medicine Trial, Encourages Access to Medicines in Developing Countries

The New England Journal of Medicine recently released the results of a ground-breaking trial of an HIV-prevention method called oral pre-exposure prophylaxis (PrEP). Data from the trial revealed an almost 44 percent reduction of new HIV infections among participants who took the antiretroviral tablet daily to prevent HIV, compared to those who took the placebo pill.

Participants in the study included 2,499 HIV-negative gay men, transgender women, and other men who have sex with men (MSM) from Peru, Ecuador, Brazil, South Africa, Thailand and the United States; these were randomly chosen to receive the antiretroviral (ARV) drug TDF/FTC (brand name Truvada) or a placebo pill. Regular HIV tests were administrated throughout the study. At the end of the trial, 36 participants who took Truvada had become infected, compared to 64 of the participants who took the placebo pill.

Similar trials are currently being conducted among heterosexuals in Africa and injection drug users in Thailand. Additionally, a trial of a similar ARV in gel form is currently being tested in three US cities, Pittsburgh, Boston and Birmingham, Alabama.

Global Health Progress is encouraged by the outcome of this trial, as it could lead to the prevention of HIV in developing countries and around the world. However, additional funding is needed not only to continue developing drugs like this, but to increase access to medicines around the world. As the world of medicine continues to advance, it is the responsibility of the entire international community to facilitate access to medicines. Success depends on all sectors working in partnership; not only to make medicines more accessible, but also to ensure continued innovation into new medicines for the treatment and prevention of all diseases.

Overweight Struggle With Weight Loss Because Of Heightened Sense Of Smell

New research has found that some overweight people have a greater sense of smell for food which may cause them to struggle with diets and weight loss.

Carried out by the University of Portsmouth , the study found that those who were overweight, or had a high BMI, had a far heightened sense of smell for food compared with slim people.

Volunteers took part in the study where they were asked to join in with a series of mini experiments that tested their smelling ability.

The results revealed that most people are better at detecting food odours just after they had eaten rather than when they were hungry with experts citing this as one of the reasons many people continue eating long after they are full up.

Scientists believe this new research could help explain why some people struggle to stay slim or stick to a strict diet.

Lead researcher Dr Stafford, said: “It could be speculated that for those with a propensity to gain weight, their higher sense of smell for food related odours might actually play a more active role in food intake.

“Hopefully this research will stimulate more work in this area with the potential to help those who struggle with their weight and those who treat people with weight problems.”

South Florida Dentist Warns about Holiday Foods

When most people think of foods that can harm your teeth, they think of candy. “However, there are a lot of other foods to avoid,” says Dr. Charles Nottingham, a cosmetic dentist at Smile South Florida. “Unfortunately, many of them are popular during the holidays.”

Sugar: Sugar, of course, is the #1 culprit in causing cavities. The worst offenders are sticky foods like lollipops, hard candies, and gum drops. They give you a double whammy. Not only are they high in sugar, but they also stick to your teeth for a long time after you eat. Surprisingly chocolate is less like to be a culprit because it binds with fat molecules and doesn’t stick to teeth as well as other candies. If you do eat a lot of sugary foods, try to do so at mealtimes when increased saliva can wash sugar off teeth.

Starches: “Bread and potatoes are very popular parts of traditional holiday meals,” points out Dr. Nottingham. “But, the refined flour in white bread and the starch in potatoes are essentially sugars and so will affect your teeth the same way refined sugar does. So cut back on those rolls, mashed potatoes, holiday cookies, and potato chips.”

Alcohol: An occasional drink at a holiday party is okay. But excess alcohol wears away tooth enamel, discoloring teeth and making them vulnerable to cavities.

The good news is there are a lot of foods that are healthy for your teeth as well. “In general most fruits and vegetables help strengthen your teeth – as do a lot of foods containing protein,” says Dr. Nottingham.

Fruits and Vegetables: Any fruit or veggie with a lot of fiber will help out. Onions, celery, kiwis, parsley, and apples are all singled out by experts as being particularly good for your teeth. So look for holiday recipes that include these foods.

Dairy Products: In general cheese, yogurt, milk, and other dairy products (without added sugar), are good choices for the teeth. They help generate saliva which washes sugars off the teeth and they contain calcium, which strengthens them.

Florida Gynecologist Rebuts Misconceptions About Cosmetic Procedures

Cosmetic gynecology is a burgeoning field with a growing number of practitioners and patients all over the U.S. However, some doctors expressed concern about the field in a recent issue of OB.Gyn News. Many worried that such procedures aren’t medically indicated, while others were concerned about negatively affecting a woman’s self-image by suggesting that her private parts might be unattractive.

However, Dr. Mark Scheinberg, a South Florida cosmetic gynecologist from the Center for Cosmetic Laser Gynecology, knows these criticisms are unfounded. “I have seen hundreds of patients in my practice, “says Dr. Scheinberg. “Many of them have functional concerns, such as pain, difficulty with urine retention, or diminished sexual satisfaction. Aging, childbirth, and other causes can result in some distressing changes in the vulva area. After surgery, they are very happy to have these functions return to normal.”

Other critics have voiced concern that gynecologists’ straying into purely cosmetic treatments is a inappropriate. However, as Dr. Scheinberg points out, “These procedures are no different than breast enhancement or tummy tucks which are performed for cosmetic reasons. Enhancing your appearance is a valid choice for women. It can improve their self-image and quality of life.”

Ultimately, Dr. Scheinberg notes, it’s an issue of giving women choices. “I believe that women should have cosmetic gynecology options,” he says. “It’s not up to me to tell a woman whether she should or shouldn’t be happy with the appearance of the vulvar area. It’s the patient’s choice.”

Global Health Progress Amplifies New Report that Outlines Barriers in Securing Access to Medicines in Least Developed Countries

“There is a public health crisis in the least developed countries. It is a crisis made by humans and solvable only by humans. There is urgency for governments, businesses, NGOs and academia to collaborate in dealing with this most urgent of problems,” declares Cameron Institute’s new 200-page report, Pharmaceutical Access in Least Developed Countries: on-the-ground barriers and industry successes.

Executive Director D. Wayne Taylor, Ph.D., F.CIM., who prepared the report, cites the recent World Health Organization’s (WHO) Draft Global Plan and Strategy for Action in stressing the importance of pharmaceutical industry partnerships. The report states that the industry must focus its energies on “the growing burden of diseases and conditions that disproportionately affect developing countries, and particularly women and children.”

Global Health Progress actively supports the report’s outlined actions needed to solve this issue, including endorsement of public-private partnerships (PPPs), protecting intellectual property and drug patents as the cornerstones of affordable innovation, improving access to medicines and developing new drugs.

Public-private partnerships are one key way to improve access to medicines in the least developed countries. As the report notes, the research-based pharmaceutical industry, in various public-private partnerships, has been improving access to medicines in the least developed countries of the world since the 1950’s. To date, there have been 150 public-private partnerships whose aim it is to improve access to medicines and currently 90% of PPPs are industry-led.

Additionally, innovation is cited by the report as “the hallmark of the pharmaceutical industry. Without innovation there would be no immunizations,
medicines or biologics to save lives, improve quality of lives, and to save much needed healthcare dollars. A basic right in law is to retain possession of one’s own ideas and discoveries.” The report also notes the importance of protecting this innovation through intellectual property and drug patents, as well as through the infrastructure and legal structures needed for healthy growth.

The report adds that every dollar invested in new medicines relieves the healthcare system of seven dollars of expenditure elsewhere, and that new drugs increase life expectancy and life-time income by about 1.0% per year while also decreasing years of life lost. It also provides a summary of 2010 data regarding contributions provided by research-based pharmaceutical industry to least developed countries:

• USD 8,450,000,000 of product and investment
• 950,000,000 patients treated
• 9,500,000,000 doses/treatments administered
• 350,000 doctors, nurses and other health personnel trained

Despite this progress, actions are still needed to solve the problem without creating additional difficulties. For example, even though existing medicines for HIV/AIDS has reduced mortality rates in developed countries by 70%, adherence and compliance are two major obstacles, amongst many, to the successful deployment of these drugs in developing countries. Additional barriers to access and improved health in least-developed countries include “on the ground” barriers, such as market failure, corruption, non-existent health human resources and infrastructure, and the lack of both local and international political will.

Fact Sheet Reveals Mexico’s Increasing Investment in Biopharmaceutical Sector

A recent Global Health Progress fact sheet highlights how Mexico’s investment in its biopharmaceutical sector has spurred economic growth, fostered global competitiveness, reduced the health and economic burden of disease on local citizens, and improved access to medicines. With the tenth largest economy in the world, Mexico is currently investing more than 1% of its gross domestic product into its biopharmaceutical sector and this percentage is anticipated to increase as the country works to develop tomorrow’s medicines.

Like the U.S. and other big emerging markets, Mexico has recognized the potential of the biopharmaceutical sector to be an important source of economic growth through the creation of high-quality, high-skilled jobs. Currently, Mexico’s biopharmaceutical sector employs nearly 25,000 Mexican nationals at more than 32 biotechnology companies. The number of skilled workers entering the workforce is predicted to increase as more than 750,000 engineering and technology students in Mexico are currently enrolled in local universities. Additionally, collaboration between the public and private sector has resulted in the creation of more than 100 research centers.

Mexico’s emphasis on job creation stimulates the developments of new drugs, which improves access to medicines for its entire citizenry. For example, there are approximately 254 medicines in development or awaiting regulatory review for Mexican patients and Mexico currently ranks 19 out of 25 in terms of active clinical trial sites. The growth in share of global clinical trial sites can bring health benefits to Mexican patients, such as diffusion of medical knowledge, greater patience access to medicines and high quality care and effective medical practice.

Recognizing the potential for economic growth, improving access to medicines and inspiring drug innovation, the Mexican government has sought to implement policies to foster the growth of the sector. The National Council for Science and Technology, the principal government funding agency for scientific research in Mexico, provided approximately US $16 million to fund basic science projects between 2001 and 2004. Mexico also has several bioclusters, made up of biotechnology, biopharmaceutical and related companies, focused on pharmaceutical R&D. In addition, public-private partnerships between the Mexican government, academia and industry are increasing due in part to the government’s continued commitment to science and technology research and education.

Typically, 7 years ago, 46% of Breast Augmentation patients went up three cup sizes whereas over the last 12 months THMG’s most popular size increase was 2 cup sizes. Breast Surgery remains the most popular of all cosmetic surgery procedures, and it accounted for 48% of all procedures undertaken by The Harley Medical Group in the last 12 months. Irish attitudes have changed dramatically from 7 years ago, as women are now happy to show-off their surgery. Over 65% of Breast Augmentation patients are between 25-35 years old with the balance mostly post breast feeding.
