NC Plastic Surgeon Discusses Importance Of Breast Implant Placement

While the issue of ‘over’ or ‘under’ the pectoralis major muscle receives a great deal of attention, even more important than implant position relative to this muscle is implant position vertically and horizontally on the chest wall. In many patients, the inframammary fold needs to be lowered in order to allow the implant to rest at a level that appears natural relative to the position of the nipple and areola, and in order to prevent the appearance of excessive upper pole fullness. In my Raleigh North carolina Plastic Surgery Practice, I regularly place breast implants below the muscle and above the muscle.

In profile, the natural-appearing breast is not convex in the upper pole, and an excessively convex and overly full upper pole is a dead giveaway that an implant sits below the skin. Likewise, if the inframammary fold is lowered too far, the augmented breast will appear ‘bottomed out’, with an excessively full lower pole, an empty upper pole, and a nipple/areola that appears to sit too high on the breast – another situation with a distinctly unnatural appearance.

The horizontal position of breast implants also requires a great deal of attention, both in pre-operative planning and in the operating room. Breast implant pockets that extend too fat laterally will result in augmented breasts with an excessively wide space between them in the cleavage area, and the appearance that the breasts are abnormally far apart. If the pockets do not extend far enough laterally, however, the result is an augmentation with an abnormal ‘side by side’ appearance. As it is the lateral projection of the breasts beyond the lateral limit of the chest wall (in frontal view) that, along with the concavity of the waist profile and the convexity of the hip profile, produces the appearance of an ‘hourglass figure’, careful attention must be paid to ensure that lateral breast projection is adequate and appropriate.

Another consideration is that the implant base diameter must match the existing anatomic limits of the breast preoperatively and the breadth of the anterior chest in general. Obviously, a given implant volume and diameter that works well for a small-framed patient who is 5’2” will be inadequate for a large-framed patient who is 5’10”. Careful evaluation of all of these issues is necessary if the ultimate goal of the surgery is a natural-appearing breast enhancement.

I urge all propective patients to take time, do a lot of research and view many many before and after photographs before selcting a plastic surgeon to perform your breast augmentation. Taking some taime to speak to several former patients who have had the same surgery you are considering can also be helpful.

Restylane and Perlane are naturally-occurring injectable agents used for lip enhancement and to soften facial lines and creases. We have found Restylane, Perlane, Juvederm Ultra, and Juvederm Ultra Plus to be very well tolerated, and have yet to encounter an allergic or inflammatory response to any of these products. According to our Allergan representative, our Raleigh, North Carolina Plastic Surgery Practice is one of the top users of Juvederm in the U.S., but we do still use Restylane and Perlane for some patients and we feel confident in the safety and efficacy of both Restylane and Juvederm products.

Restylane and Juvederm do NOT contain any material that does not naturally occur in soft tissues, such as hydroxyapetite or methylmethacrylate, which can result in granuloma (cyst) formation and chronic inflammation. Be VERY wary of injectable agents that contain materials that do not naturally occur in soft tissues, as the risk of these complications is often minimized to patients.

Restylane or Juvederm injections require just a few minutes to perform. The aesthetic improvement is immediate, and in general we will attempt to slightly overfill the lips and overcorrect lines and folds, so that the desired effect remains once the mild initial swelling subsides (in about 24 hours). The goal is an aesthetic improvement that appears natural.

Most injections of Juvederm Ultra or Juvederm Ultra Plus, Restylane or Perlane can be performed with little or no pain by administration of local anesthesia just prior to the injection. Injection of the lips is usually completely painless, using intra-oral anesthesia that produces total numbness of the lips. For injection of lines and creases on the face, a local anesthetic solution is used that causes vasoconstriction, decreasing the likelihood of bruising following the injection. Our aim is to provide an immediate and beautiful aesthetic enhancement, with little or no discomfort and downtime.

The duration of the improvement varies from patient to patient. Some patients have reported ressults of up to a year with Juvederm Untra Plus. Many patients that enjoy the results of their hyaluronic acid treatments become interested in permanent soft tissue enhancement. For these patients, structural fat grafting, using autologous (the patient’s own) fat. Using specialized instrumentation and technique, structural fat grafting can produce permanent improvements in facial contours.

Prevelle Silk is a new hyaluronic acid dermal filler that can eliminate wrinkles or create fuller lips instantly. Dr. Michael Law www.michaellawmd.com has recently introduced Prevelle Silk into his North Raleigh Plastic Surgery Practice. Prevelle Silk contains lidocaine, a numbing agent that helps minimize discomfort associated with the injection. Prevelle is also unique because it leaves virtually no swelling. People can get an injection in the afternoon and look beautiful just hours later. The cost for Prevele Silk is just $295.00 for a 1 cc injection. results lasts approximately 2-3 months.

I expect that many people who try Prevelle Silk as their first filler will be pleased with the results and may opt for a longer lasting filler like Juvederm, Perlane or Restylane for longer lasting wrinkle reduction or lip plumping in the future. Mentor Corporation, the manufacturer of Prevelle Silk will be introducing other hyaluronic acid based dermal fillers in the near future that promise to have a longer lasting results than Prevelle Silk.

Evolence is a porcine (Pig) derived collagen product used to provide soft tissue augmentation. Although no allergic reactions have been reported with Evolence, there have been several reports of nodule formation in the lips. I am currently not using Evolence in my practice. I feel more comfortable with Juvederm, Restylane or Prevelle Silk for temporary soft tissue augmentation and structural fat grafting for permanent augmentation.

A number of other products are available as soft tissue fillers, but many are as yet inadequately tested, and others have been found in some cases to produce disastrous and permanently disfiguring results, such as injectable silicone. Remember, when considering the injection of a soft tissue filler, be sure that your doctor can explain to you what the substance is, why he or she has chosen it, and what side effects and potential complications can occur. Once again, be very wary of injectable agents that contain materials which do not naturally occur in soft tissues, as the risks of complications can be significant. The Harley Medical Group has revealed that while breast surgery is proving to be hugely popular, many women are opting for a more natural look than the all-up-front look sported by Page 3 and glamour models

Nurse counsellors across the Harley Medical Group‘s 23 UK clinics questioned women who’d recently undergone breast surgery during, page three model, Nicola McClean’s tenure on celebrity reality TV show, “I’m a Celebrity, Get Me Out of Here”. Women were asked what they think of Ms. McClean’s self proclaimed “lovely jugs” with the majority of those questioned giving the glamour model’s chest a firm thumbs down.

Of 121 women questioned, 99 (81%) said that they though her breasts were just too overblown and artificial looking. Instead, the women said they would opt for a more natural look, reflecting the ‘less is more’ attitude now taken by other celebrities including Jordan and Ulrika Jonsson.

This attitude is backed up by figures from The Harley Medical Group, which show 75% of women who have had a breast enlargement in the last six months opted to go up just one or two cup sizes. The figures from the Harley Medical Group also reveal that boob jobs remain the most popular of all cosmetic surgeryprocedures, making up 30% of all procedures across its 23 clinics.

Director of the Harley Medical Group, Liz Dale said: “Breast enlargement has always been top of the ops we perform. It’s a very personal choice and if a ballooning bust is what Nicola wants then good luck to her. However, over the last five years we’ve really noticed that huge breasts are out and instead women come to us looking for a natural result, not a ‘look at me’ statement neckline, which can sometimes cause too much attention.

“In fact, the most popular cup size requested is a C cup and 80% of our patients opt for just a one or two cup size increase. Contrary to popular belief, most cosmetic surgery patients want to fit in – not stand out.”

The new breed of breast enlargement patients, keen to keep things in proportion, are plumping for the “lower profile implant” in preference to two other available breast implant shapes – the “round high profile”, which produces a more rounded appearance to the upper breast and the “contoured” breast implants, which are designed to reflect the slope of the breast.

Sarah McLaughlin, a Harley Medical Group patient from Leeds, had a breast augmentation, increasing her bust size from 32A to a 32C. She said: “I couldn’t stand my flat chest and I’d planned to get a boob job for a while but I was wary of it looking too obvious – particularly as there are lots of men in my line of work. A C cup is perfect for me, giving me a sexy but subtle shape. Now I can dress my boobs up or down, depending on the occasion.”

Dear Food,

I’m breaking up with you.

Don’t be surprised. I’ve certainly tried to do it many times before, but I always weakened and went back to you.

I finally woke up and realized this relationship is not good for me. I’m not getting what I need from you. It’s hurting me, not allowing me the space to grow in the ways I need to grow. You’re holding me back.

I know I’ll have separation anxiety, but I’ve stayed with you too long, way past when I should have. This just isn’t working for me. You don’t listen to me. You don’t give me attention. You don’t care about what I’m going through. It’s almost like you don’t have any feelings for me at all, yet I’ve stuck close to you, helpless and dependent.

I know you won’t let go of me so easily. I know you’ll keep calling me, asking me to come back. But please, let me go now.

In the beginning you were always there, ready to calm me and soothe me, but it went too far. I came to rely on you. I looked to you for everything. In all fairness, it was too much to ask of you. No way could you fulfill all my needs. For so long I’ve settled, afraid to go out there and find something better for myself. I need to do that.

I now withdraw my heart from you so I can be free to put it elsewhere, some place where I can be loved back. I’ve tried to break up with you before. This time it’s real. I want to be on the cutting edge of my own life, and I can’t do it while I’m still tied to you. Thinking about being free scares me, but I want it. I must have it. I think I’m brave enough at last.

Field Fisher Waterhouse has recovered compensation for the cost of a claimant’s hospice care in a case that is though to be the first of its kind

Field Fisher Waterhouse (FFW), the full service European law firm, has recovered the value of the hospice care that was required by a claimant. This is thought to be the first case of its kind, with most hospices generally exclusively funded by charitable donations. As part of the settlement, the firm was able to recover £12,500 to be paid over to the hospice.

The case arose out of the death of a 64 year old man due to mesothelioma, an asbestos-related cancer, in December 2007 after spending his final days being cared for in a North London Hospice dedicated to cancer patients.

Through his life the man suffering from the cancer had worked as a carpenter, painter and labourer. In 1963, whilst employed as a painter for J Murphy & Sons Ltd, he was exposed to asbestos dust whilst sanding down and filling in walls in prefabricated housing containing asbestos. He was then exposed to further asbestos when working for the same company at a factory in Dagenham, cleaning debris from the guttering on roofs made from corrugated asbestos.

In August 2005 he was diagnosed with mesothelioma, an asbestos related cancer on the lining of one of his lungs. He was eventually admitted to the hospice where he spent the last month of his life.

Rodney Nelson-Jones, partner in the Asbestos Claims Group at Field Fisher Waterhouse, was instructed to claim compensation on behalf of the man’s family. The firm was able to recover £170,000 from J Murphy & Sons Ltd, £12,500 of which is being paid to the hospice which provided him with care worth over £400 a day.

This is thought to be the first time that compensation has been recovered for a patient’s hospice care, which is usually funded by charitable donations. It has long been possible to recover compensation for the value of care provided by relatives but FFW’s innovation has been to extend this principle to charitable care provided by hospices. The firm plans to continue to claim the cost of hospice care in future mesothelioma cases.

Rodney Nelson-Jones commented: “We are delighted to have been able to secure extra funding for the hospice. Hospices do a wonderful job and rely on charitable donations to provide much needed palliative care for cancer patients. We hope that this case will mean that they are now provided with an additional source of funding.”

According to the Centers for Disease Control and Prevention, over nine million children in the United States are now considered overweight or obese. But as more and more adults turn to various forms of surgery to deal with their obesity, the question arises whether or not the same medical procedures are suitable for children.

Officials From East Carolina University And University Health Systems Of Eastern Carolina Dedicated The East Carolina Heart Institute Today

Officials from East Carolina University and University Health Systems of Eastern Carolina dedicated the East Carolina Heart Institute today. Several hundred invited guests joined leaders from the two organizations to celebrate the facility. The Heart Institute puts ECU and Pitt County Memorial Hospital “at the forefront of progress and the cutting edge of the future,” said Dr. W. Randolph Chitwood, director of the East Carolina Heart Institute.

“What we dedicate today is a concept called the East Carolina Heart Institute that encompasses these new facilities, dedicated to the people of this region, and to the physicians, researchers, educators and staff devoted to their care,” said Chitwood, who is cardiothoracic surgery and vascular surgery at the Brody School of Medicine at ECU. He is also senior associate vice chancellor for health sciences at ECU.

The dedication ceremony, held at the East Carolina Heart Institute at ECU, capped more than four years of work to bring a world-class cardiovascular disease institute to eastern North Carolina. In 2004, the General Assembly approved $60 million for a research, education and outpatient care facility at ECU. Pitt County Memorial Hospital secured private funding for a $160 million bed tower. Today, leaders from the two organizations celebrated the end of construction on both facilities.

Many who attended the ceremony also toured the two facilities after the event ended. The crowd included local and state officials, donors and other friends of each organization.

The celebration marked “the most significant collaboration University Health Systems and East Carolina University have ever undertaken,” said UHS CEO Dave McRae. “It’s the biggest step of a journey we started more than 30 years ago, when a small county hospital and a fledgling medical school committed to forming the world-class academic medical center we’re part of today,” McRae said.

ECU and PCMH have been “joined at the hip” for decades, ECU Chancellor Steve Ballard said. “The East Carolina Heart Institute is the latest partnership between us, and it will make a major and lasting impact in eastern North Carolina, the entire state and beyond,” Ballard said.

The new buildings aren’t the only sign of a new approach to heart disease for ECU and PCMH. The two organizations have also redefined their model for treating cardiovascular illnesses. Both have organized their clinical staffs around illnesses and disease processes, rather than following a traditional model based on academic specialties. That new model encourages more information-sharing among doctors and puts new emphasis on patients’ best interests, Chitwood said. “This new model of care delivery is an innovative way to blend the science and art of medicine so that we add value to the patient,” Chitwood said.

The prevalence of cardiovascular disease in North Carolina justifies the massive investment in treating and researching heart and vascular illnesses. Cardiovascular disease is the second-leading cause of death in the state, and nearly a quarter of N.C. residents suffer from cardiovascular ailments.

Planning for the East Carolina Heart Institute dates to early 2003. ECU and PCMH held a groundbreaking for the Heart Institute in March 2006. In addition to the six-story 375,000-square-foot bed tower, the Heart Institute at Pitt County Memorial Hospitalspawned a pair of companion projects: construction of a massive central utility plant to power the new building and the relocation of Moye Boulevard.

The Heart Institute at Pitt County Memorial Hospital has 120 cardiovascular beds, six operating rooms, seven interventional laboratories, three electrophysiology labs and a heart-healthy cafeteria, all designed to create the optimal patient care environment.

The four-story, 206,000-square-foot East Carolina Heart Institute at ECU houses science and clinical research, robotic-surgery training, future space for simulation laboratories, a clinical outpatient facility for cardiovascular diseases, a database center, offices and an auditorium.

Pruhealth Vitality Index reveals clear gap between Britons’ perceptions of what it means to be healthy and having a real understanding of the impact of lifestyle

The new PruHealth Vitality Index has revealed a clear gap between Britons’ perceptions of what it means to be healthy and having a real understanding of the impact of lifestyle, diet and fitness levels on current and future health. The Index – which will be repeated on a biannual basis to track the nation’s health trends – provides a comprehensive analysis of general health and wellbeing in Britain.

The report reveals that 69%* of people in Britain claim they are in a good state of health. However, when lifestyle and fitness choices are scrutinised the reality paints quite a different picture as a significant proportion of those who feel they are in a good state of health do not follow a healthy lifestyle:

– only 58% feel they actually follow a healthy lifestyle
– 47% describe their fitness levels as ‘good’
– 24% eat their recommended five a day of fruit and veg
– two thirds think they are overweight
– 21% of Britons smoke
– only a fifth of Britons don’t feel under any stress
– only 14% go for regular screenings or check ups with their GP

Dr David Grainger, Senior Fellow, Cambridge University, said: “This report makes clear that people listen to and understand the public health message. Lifestyles are improving and disease rates are falling. But there is plenty more to do.”

He continued, “If half of current smokers managed to stop smoking tomorrow, we estimate that nearly a million years of lost life would be saved from heart disease alone. Regular health checkups are particularly useful, and it is disappointing to see that only 14% of respondents had regular health checks, despite the emphasis of the public health message on prevention rather than cure.”

Dr Grainger believes,The Vitality Index is a valuable new indicator of attitudes and behaviours that promote a healthy lifestyle. The Index balances the importance of a number of different areas to provide an interesting snapshot of lifestyles across the UK, and has the potential to predict future trends in public health. This focus on balance, rather than over-emphasis of the importance of one particular element of a healthy lifestyle, conveys exactly the right message at a time when we are in danger of being overwhelmed with guidance on staying fit and healthy.”

The majority (81%) of Britons say they know what they should be doing in order to follow a healthy lifestyle, but many simply aren’t putting this into action, with lack of motivation, cost and lack of time clear barriers to living a healthy lifestyle. More than half (53%) of respondents feel free access to exercise and swimming classes would help tackle national obesity, and 49% claim cheaper fruit and vegetables would have an effect.

*The research was carried out on behalf of PruHealth by Ipsos MORI. A nationally representative sample of 3,034 GB adults were questioned online in July 2008. Results are weighted to be representative of the GB adult population.

Continued testing in rural Southern Africa shows promiseful results for those with HIV/AIDS

AIDS Research Assistance Institute, a non-profit organization, has been monitoring hundreds of people with AIDS/HIV when they add the immunity boosting food supplement, ‘concentrated Flax Hull Lignans’ (FHL) to their diets. The results are better than expected and may hold a key for future researchers as they try to find solutions to the AIDS pandemic worldwide.

Boasting a 92% success rate, A.R.A.I. has been involved with rural Southern African grass roots organization leaders who have been giving FHL to people suffering from the AIDS/HIV virus. Typically when an HIV infected person adds 1.5 tsp of the FHL to their daily diet, their viral loads drop significantly (around 35% of them drop to non-detectable within the first 6 weeks!) and their CD-4 counts (measurement of immunity) raise in great numbers. A malnourished child will also typically start growing in height and weight once introducing the FHL into their diets. Many children have grown between 6-11 cm in a three month period!

Researchers can only speculate as to the exact function of the FHL that is causing the success. Many studies have been done in the U.S. by doctors that show lignans boost immunity. The FDA has reported that flax seed lignans have anti-viral, anti-bacterial, and anti-fungus properties.* Tests have shown that FHL’s ORAC values (measurement of anti-oxidants) are very high. Kale, the super anti-oxidant dark leafy green vegetable, has an ORAC value of 1770, while FHL’s equivalent ORAC value is 19,600. FHL has typically been researched for it’s effects against cancer but now hundreds of people with AIDS/HIV are feeling better, going back to work, and are causing researchers to take a serious look at the possibilities of using a simple, all natural food supplement to help fight AIDS/HIV.
